About RR Care Management
We provide personalized services centered around continuity of care, accessibility, and unnecessary hospitalizations, all while maintaining compassion for every one of our patients. Figuring out how to care for your aging or disabled adult family member can be difficult and frustrating. RR Care Management can serve as a support base and resource along the way.
RR Care Management understands the importance of a global system (a multidisciplinary team that includes working with medical providers and community providers), so we consistently strive to create an inclusive approach that put your family member’s needs 1st.
what is Care Management?
According to the Aging Life Care Association, care management is defined as "a holistic, client-centered approach” to working with families to provide individualized services for their aging and disabled loved ones. Care Managers help provide guidance towards actions and decisions that ensure quality care and an optimal life for the aging and disabled populations. Individualized services include assessment and monitoring, planning and problem-solving, education and advocacy, and family caregiving assistance.

Kizzy Chambers MSW, LCSW, C-ASWCM, CMC, is the Founder and Owner of RR Care Management, LLC in Orlando, FL. She is a Certified Care Manager and a Licensed Clinical Social Worker. She has over 13 years of experience in health care, as a care manager, and working in hospital settings. She is also a mental health therapist, focusing on depression, anxiety, stress management, and crisis stabilization. She primarily works with older and disabled adults.
Kizzy attended the University of Central Florida, where she obtained her bachelor's degree in Psychology and her master's degree in Social Work. She is active with the Aging Life Care Association (ALCA) and currently serves on the ALCA Florida Chapter's board of directors. She also serves as the Orlando Unit Leader for the ALCA Florida Chapter. Kizzy served on the 2020 National ALCA Conference Committee. She is a member of the National Association of Social Workers (NASW).
Kizzy enjoys assisting older adults, and their family members navigate the complex medical system and resources, advocating for continuity of care, self-preservation, and dignity for older and disabled adults. With her knowledge and experience as a Medical Social Worker, she understands the importance of working within a family system and mitigating family conflict.
When Kizzy is not advocating for her clients and their family members, she enjoys time with her family. She and her family love to go to the beach and have family barbecues.
